ROF2 client for sure.
Only way to fix it is to zone or camp that I've found.
Doesn't seem to really be a rhyme or reason to why it does it sometimes it does sometimes it doesn't.
Only happens when swapping an item though.
If i ask them to give me an item manually using inventory remove it works fine.
