Default Phantom mobs

We've been having an issue with "phantom" mobs on our server for a couple of months now.

What we're seeing is an occasional mob showing up, and can be clicked on, but is not really there. Similar to the old (and occasionally still an issue) problem where a bunch of mobs would appear to be at zone in or some other safe spot but would actually not be there.

The last two of these occurrences happened yesterday, both on deaths. The client that died zoned back in and all the mobs that had trained all showed as ghosts, where they were when that client died. Other clients correctly show the mobs back at their home locations.

Ideas?

Both of these issues should have been addressed in a recent round of source changes

The one specifically with mobs ghosting at the entrance I confirmed having fixed recently.

Ghosting was a symptom of selective position updates but we have also entirely changed that with the recent movement overhaul so I would make sure we're talking about recent code
