Cosmetic Question - Item icons

I was working with some items and noticed that the ingame icon doesn't match the item in question, however the rendered graphic is correct and the stats all get applied.
Example:
I summon up a sword and equip it, the stats are applied, the character is holding a sword however the icon in the slot is a necklace, even in the info window. I have not changed or updated the database since I installed it. I have looked at the item table and compared it to EOC and the values for icon and idfile match. Is there a way I can fix this? It's just weird seeing a necklace icon in slots for legs, swords, chest, and arms.

The server was built with Akkas all in one installer, and my client is the Titanium Edition.

It's the client. The item has a newer icon than the client has, this causes the icon to show as a Pearl Necklace as that is the default when the client can't find the proper icon. To fix you'll have to either change the icon the item displays to something older, or use a newer client.
