Go into your DB and delete the Account Trelosx, should fix your problem. If you have toons under this name, just change their Account ID to whatever is created by your next login.
This is ASSUMING that you have added all the new SQL additions in the changelog to the database as well.
Whoops just re-read your post. Delete YOUR Account Name in your DB and see if you can login and of course, the changlog SQL additions still apply. The error above is stating that the name exists but it is trying to duplicate it so some of the info is not adding up.
