I had the same thing doing the combine for the Iskar Hermit in WCab, the final combine for Ixpacan, and, oddly, on turning in the item that spawns the Child of Charasis. This was on a SOF client running on Wine on Ubuntu.
The child was still there when I logged back in, and I got my clicky hat (YAY!!!).
I wonder if the client received a data structure that was the wrong size.
