I don't know how old your code is..but, I made some pretty broad changes to inventory variable references recently.
Is your code newer than the last couple of weeks?
I did test all of the slot ranges on all of the supported clients before committing it..but, that doesn't mean that a typo didn't occur somewhere.
Post the item ID of the SoW potion that you used and I'll see if I can replicate it.
