I did a fresh install following the guide provided here:
http://www.eqemulator.net/wiki/wikka.php?wakka=GetTit

When it wouldn't work I followed the section of the FAQ found here:
http://www.eqemulator.net/wiki/wikka.php?wakka=EQEmuFAQ#QLinkL

PC Specs
Case - Antec Sonata II
Motherboard - Gigabyte GA-MA770-UD3
Processor - AMD Athlon II X4 620 4800+
RAM - Corsair DDR2 6GB: 4 slots, 2 2GB, 2 1GB
Video Card - ATI Radeon HD Asus EAH4870 DK/HTDI/1GD5
Operating System - Windows 7 Home Premium 64-bit
Drives - DVD-RW, DVD-DL writers
Power Supply - Corsair tx750w
Displays - Samsung 21 inch wide screen, LG Flattron L222WT 22 inch wide screen

So I get the black screen with a mouse cursor, when I try clicking the game blinks and closes, sometimes I get an error message saying "devicereset".

I have tried run as administrator, various compatibility options, resolutions as well.

Did you install directx9c? And no, it does not matter if you have dx11 as part of Windows 7. Installing directx9c will not overwrite dx11 it merely adds the runtime components, some of which are required by Everquest.
You can download dx9c from http://www.microsoft.com/downloads/en/details.aspx?familyid=2DA43D38-DB71-4C1B-BC6A-9B6652CD92A3&displaylang=en&Hash=j0Zh1cSQXtbknX9RwKYa55NxhcjvXOd3%2B%2B5WaCJmo RLPkJ6dydzdeuTo9Ii0Zxv0iCFWsqUmFMKPIKV008JDDg%3D%3 D

The devicereset error is almost always a driver issue or a failing gpu.

I get a similar problem, check if you can press ALT-ENTER to get out of full screen mode and get into windowed. I have noticed the startup screens have issues on some computers, especially newer ones. once you actually in the game, switch to full screen if you want, but you may get the gfx card couldn't reset, I get that sometimes too, about 25% of the time.

If you using the titanium client, you may want to use the SoF one as its more recent and I'm sure has less problems (Im using titanium too)

Thanks for the quick response, I installed the driver you suggested but unfortunately it didn't make a difference. The exact error I get when I get it is "devicereset()" if that helps at all.

This did the trick thank you!
