I wrestled with this quite a bit and tried all manner of posted solutions (including modifying resolutions and using Rogean's modified dsetup.dll), but I simply could not get it to work. I am running Windows 7 64-bit with HD 4200 integrated graphics. Here's what I did to finally get it. Note that this problem seems to be caused by a number of different scenarios, so this may not be the solution for you.

Download the redistributable for DirectX9. All you need is the "dsetup.dll" from this, but I didn't want to spook anyone by providing a link from a non-official source.

Unpack it and nab the dsetup.dll (it's extremely small relative to the newer ones) and replace the one in your EQ directory. A problem I ran into is the EQEmuLauncher seems to replace this, even if you have patching turned off. So, I had to use a direct shortcut (only reason I was trying the launcher was because of the problems I was having).

Now modify your eqclient.ini to include "WindowedMode=TRUE." Note that this is one of the recommendations from other posts, but this alone did not solve my issue.

This may get you going if you are one of the unfortunate few for which the other solutions do not work.
