Killed by an unseen hand

Anyone ever seen this before? There's no NPC by this name, there's no scripts spawning mobs, there's no hidden mobs in the zone, etc. It's happened twice to this person and I can't replicate it. Nothing shows up in the combat log but the character is losing HP fairly rapidly. And then just dies.

Could it be the spell the ghouls are casting?

Would that not still show up in the combat log? Those wizards casting in his log aren't fighting him, they are just close enough for him to see the casts. IIRC, he was only fighting a zol ghoul.

It's a client generated message, triggered when you being killed by something that for some reason is not on your entity list (most typically an unseen GM).

I am the GM, so I know it's not me. Like I said there are no hidden NPCs in the zone.

Didn't think of that, I'll check and report back.

Edit: I can't get this to replicate either. If mob dies during casting he stops. If mob dies after casting the effect stops (no more ticks). I can't get the npc to finish cast while also dying.

Edit2: Also confirmed when this happened the first time no enemies were engaged. User had just came back from a long afk and was sitting in a corner with ITU on.
