the most evident would be that it is not able to see the mysql server. which version of it are you running? i had some issues using the newest version of it. currently i'm running the 3.23 production...

to find out what errors yer getting, use a command prompt in windows instead of just running the program.

START > Run > Command > OK

then goto your EQEmu directory and run world.exe from...
