I can't make accounts in my database

hey i keep getting this error cannot open account.myd i have tried starting fresh and everything and i still cant get it to work i dont get errors or anything until i try and log into my server when i try and login my world.exe gets a bunch of errors about not being able to open that file so if someone could please help me i would really appreciate it thanks..if you need my info just let me know but i dont think that has anything to do with it since there are no errors except that one
