I've been banging my head against the wall with this same problem for the past two days. I have tried 5 different client versions, none of them work. I then tried hybrid versions (you can many times swap out eqmain.dll with another version) and that way got it to work once, but stupid me forgot what combo of files I used. At this point I am fairly certain it's a code issue. I'm looking into now.
