I had the exact same problem a while back. The only way i was able to fix it was by downloading the 3.13 DB addon and sourcing it. For some reason that made it work.

First of all im not new to eqemu.I had it working before but now it decides it dosent want to work anymore. I start up all the world.exe minilogin.exe and boot5zones.exe crap fine without any error...
