"You have been disconnected": Usually i got this msg when I try to connect to localhost with my old server with my old 8mb graphic card. The mainboard had only 250mb/sec datatransfer. At about, it's the transfer rate of an older 440BX or a newer PLE/KLE chipset.
Although I could connect to some outdoor zones or towns, in dungeons like GUK i had lag continuous disconnections, due to the lag (or the poor datatransfer) :p .

"username/password error": hmm, the error is self explaining I think. Use minilogin and connect with std eqemu/eqemu.

If you can make a char, but you can't enter the world: do you have the supported DIRECTX version installed (actually, 8.1)? Do you meet the minimum requirements for Everquest? 64 MB aren't enough anymore, you need at least 128 MB RAM!
