Eqhost.txt

Here you go...

[LoginServer]
Host=eqemulator.net:5994

Should just be that in the eqhost.txt file... then make sure you run eqgame.exe with the parameter "patchme". This can either be done with a shortcut or through command prompt:

c:\eqemu\eqgame.exe patchme

where c:\eqemu is your everquest folder.
