Where are you trying to spawn the NPC? I know that I've had trouble in the past where I'd place and NPC and add it to the database and everything, but then it wouldn't spawn.

One thing that works sometimes is if you spawn it away from the location it should be in, do #npcspawn create/update and all that, then zone out and back in, then do a #npcspawn fix to the location you want it in.
