I think your problem is in your clients " \Everquest\RenderEffects" directory. You must have somehow corrupted some files here ( I've done this too). I don't know exactly what to do, I just have Everquest installed in multiple machines (I also keep a backup) - so when this problem began, I fixed it by deleting and coping over RenderEffects from another install.
Unless you can find your original 'RenderEffects', you may have to delete and re-install the whole thing. I now keep a copy of this too, and have had to use it a few times.
Altering your In eqclient.ini may have compounded your problem (since that wasn't the problem). for example;
[Defaults]
Sound=TRUE
TextureQuality=1
VertexShaders=TRUE
20PixelShaders=TRUE
14PixelShaders=TRUE
1xPixelShaders=TRUE
MultiPassLighting=FALSE
UseLitBatches=TRUE
Is related to your video card capabilities - I've set options to TRUE or FALSE, with results like your posted picture , or even the character was gone.
You can also try switching from old models to new , run game - then switch back. This might fix it.
