Are you running a windows server and using some older binaries? If you haven't updated in a couple of weeks, a ton has changed and continues to change almost daily. The best thing is to at least stay up to date with the windows binaries releases, and even better would be to update to the SVN source code, because that is normally a few releases newer than the binaries.
Also, you would want to do fairly regular updates, because things are still getting fixed regularly. Just keep an eye on the changelog on the SVN to see what is fixed and if it is worth updating to get it.
Oh and most importantly for your issue, I just realized that if you are having issues like that and have current server code, you almost certainly forgot to move your patch_SoF.conf file. Make sure that you move the patch_SoF.conf file from the /utils/ directory into your server directory. You have to do that with every update that updates that file or you will have certain things like the issues you are having happen. For now, it would probably be a good idea to move that over almost every time you update, as it is getting updated pretty frequently.
