I had a similar problem about a month ago when I upgraded to a new nvidia card.
How I solved it is 2 ways…
First I set the resolution to 1024x768 using the options button on the server login screen then I logged in in windowed mode.
After I got in world I then alt-entered to full screen and used the display options panel to set the res to what I wanted it to be.

That alone didn’t work for me (Crashed the next time I started on full-screen mode) so I also went to the shortcut I use to start EQ, right click it and go to Properties then the compatibility panel and check the ‘run in 640x480 resolution’ box.

That runs the launcher in 640x480 but doesn’t affect the char select or game screens.
After that it runs like a charm.

Hopefully one or both of these will solve your problem.

By the way, the Launcher screen looks like crap at 640x480 when stretched to 1600x1050.

You can go into the nvidia control panel and set it to not stretch lower resolutions and it looks normal again.
