Default HP dropping when I zone..
Whenever I zone after Equiping up, My HP drops like a rock, and I die, Then It repeats over and over again
What could the issue be?

Remove and get rid of the equipment you have. Then try. See if it makes a difference. (Hint: Possible multiple lore items)

Currently there is a bug that if u have multiple lore items that are identical and u log or zone u will get a repeating dmg bug that does around 250 dmg or so and will eventually kill the char. I think I recall trying to delete every item i had on the char and the char was still bugged. To fix this problem I had to delete the char and create it from scaratch again, which isnt difficult if u know the Emu well. If u are running ur own server then u have control over the DB and u can make any item with nearly any stats so u dont have to double up on some items and run across this bug by accident.
