Please try the "General Support" or "Win32 Support" forums next time.

Are you using EQW?
If so, make sure your desktop bit depth is the same as your EQ bit depth. To check your desktop bit depth, rightclick on your desktop, choose Properties, Settings tab, and verify 16 or 32 bit. To check EverQuest bit depth, open EqClient.ini (from your EverQuest directory) and look for the line "VideoModeBitsPerPixel=" it should be followed by 16 or 32. Both your desktop + your EQ setting should be the same.

Are you using a Debug installation of DirectX or the DirectX SDK?
If so, convert back to a retail installation (the SDK docs should tell you how to do this).
