Default Server Select screen after zoneing

Is there a fix for any of these?

1. After I perform any zoneing evolution via #zone, spell, or zone line, I get booted to the Server select screen. I can log back on and play normaly until I zone again.

2. Sometimes, a large number of MOBS are concentrated at the zone in point. I can target them and kill them but a few are mirror images of the actual MOB somewhere else in the zone. I have even seen this bug in EQ live but it's usually just one MOB in that case.

3. When in Mele Combat, the MOB is standing right on top of me. Can you tell it to back up so you can watch it get a propper ass kicking?

4. I can't see wielded weapons in my character select sreen.

The only ghosted npc's I have seen at the safespot have been pets. With 3.1.1 and my DB I zone into say Westfreeport and backup from the safe spot. In short order a few pets spawn. I can target them, but not attack. And if I do #damage 10 they poof for a moment, then come running from where ever they should have been.
