Two of the main things I think to check on a Local setup using minilogin is your EQHOST.TXT file since running either the official patch or the EQemu patch will change the file.
The other time I have ran into this error is when I had not successfully created the account. That step is missing from some of the instructions I have seen here.
Also missing from some insturctions I have seen is the need for the miniloginaccounts.ini file.
